Duties and Responsibilities
1. Provides clinic services ensuring clinic services are covered by nursing personnel.
Maintains clinic expertise and competency (including age specific competency) demonstrating thorough knowledge of procedures to deliver quality primary health care.
Documents delivery of health care and nursing processes in accordance with the Center’s policies and procedures in a timely, accurate, and consistent manner.
Educates the patient and family about diagnosis and plan of care, acting as a resource for facilitating communication when necessary.
Coordinates delivery of patient care provided by the health care team.
Interacts with personnel, assisting providers ensuring optimal patient care and optimum patient flow increasing patient satisfaction.
Identifies and responds promptly to changes in patient’s condition and/or clinic environment.
Collaborates with administrative staff, nurses, and providers optimizing efficiency of a variety of related services ranging from inpatient care to out-patient follow up care.
Ensures wait times of patients are minimal, patient referrals and prescribed medications are processed promptly, and patient confidentiality is maintained.
2. Provides clinic oversight of nursing personnel.
Supervises, advises, counsels, disciplines and evaluates nursing staff.
Ensures nursing staff coverage, scheduling, and training with the new hire department.
Ensures compliance with all standards of care, contract requirements, policies and procedures related to the clinic’s accrediting and funding agencies.
Monitors and maintains compliance of vaccines; medical equipment, and all logs ensuring all are updated and compliant.
Participates in the evaluation of products, procedures, and patient care standards.
Measures competency
Assumes responsibility/accountability for delegation of ensuring compliance with all standards of care, contract requirements, policies and procedures related to accrediting and funding agencies within the clinics are of responsibility.
Works with (CMO) in the development of standard treatment protocols for disease conditions most prevalent in the Center’s patient population.
Works with (CMO) to review and revise standard treatment protocols at least once a year to meet the medical needs of Center patients.
Participates in Compliance and Performance Improvement activities for continuous quality improvement and risk management purposes.
Serves as a resource and positive, proactive leader for the department.
Accepts responsibility for remaining current with nursing and health care trends and information impacting the nursing practice.
Participates in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care.
Is responsible for appropriate infection control and employee health programs including training staff, and monitoring compliance.
Completes medical record and nursing documentation audits and implements system changes for the nursing staff as necessary.
Travels when necessary to fulfill the entity’s needs and attends meetings as necessary to represent the Center.
Participates in the preparation of grant applications for the Center and assumes primary responsibility for developing and implementing the medical health care plan.
Assists in the coordination and integration of the corporation’s medical programs and services with other corporate programs and services for the welfare of the corporation’s patients.
Assists in the provision of technical assistance and health education in the community as requested.
Serves on the Pharmacy and Therapeutics Committee and other committees as assigned.
Demonstrating flexibility, enthusiasm and willingness to cooperate while working with others in assigned inter-professional care teams to support PCMH programmatic activities.
Participate in population management activities, as directed.
Assist the care team by helping to measure quality and identify, refine and implement practice improvements that support the medical home, and assist in performance evaluation and quality improvement.
Apply the principles of comprehensive, community-based, patient-centered, developmentally appropriate, and culturally and linguistically sensitive care.
3. Performs all other duties as assigned.
Knowledge, Skills and Abilities
Required
Knowledge of advanced principles of health promotion, prevention and motivation
Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center’s current operational protocols
Knowledge of medical terminology and appropriate abbreviations
Thorough knowledge of the Texas Nursing Practice Act
Ability to read and comprehend medical instructions, short correspondence, and memos
Ability to document, with clarity, all information relevant to a patient’s needs
Ability to effectively present information in small group situations to patients, Center staff, and the general public
Ability to prepare statistical and administrative reports
Ability to work cooperatively with community agency personnel to coordinate services for patients
Ability to use office equipment including but not limited to computers, fax machines, telephones and copiers
Ability to prioritize and allocate resources
Preferred
Bilingual in English and Spanish is preferred but not required.
Credentials and Experience
Required
Licensed Registered Nurse degree from an accredited school of nursing
Current unrestricted license from the Texas Board of Vocational Nurse Examiners to practice as a Registered Nurse (RN) in the State of Texas
Current Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals Certification
Preferred
Five (5) years of practice experience as an RN giving direct patient care
Current unrestricted license to practice as a registered nurse (RN) in the State of Texas.
Special Requirements
The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.
