Behavioral Health Case Manager (LVN)
The Case Manager (CM) is integral to the health care team. The (CM) is an intermediary between patients, providers, and healthcare institutions. The (CM) creates a care plan by using direction from the provider and input from the patient (family). The (CM) documents the care management plan in the patient record, updating progress and revisions. The (CM) serves the patient (family) as an interdisciplinary team coordinator, clinician, consultant, advocate, liaison, and educator. The (CM) follows up with the patient by making clinic, home, and phone contacts. The (CM) helps patients understand their conditions and treatment options. The (CM) reduces barriers the patient may encounter between service systems.
Chief Executive Officer (CEO)
Reporting directly to the Board of Directors, the Chief Executive Officer provides visionary leadership while ensuring operational excellence, financial sustainability, regulatory compliance, and exceptional patient care. The CEO leads a multidisciplinary executive team responsible for advancing strategic priorities, cultivating organizational culture, strengthening community partnerships, and positioning the organization for continued growth and innovation. The successful candidate will inspire people, build strong relationships, make sound decisions, and effectively navigate the rapidly changing healthcare environment.
Dentist
The Staff Dentist is responsible for delivery of quality dental care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her dental practice.
Employee Health Nurse (LVN, RN)/Training Coordinator
The Nurse Educator/Trainer is integral to the health care team by working closely with the Nursing and Human Resources (HR) Staff. Responsible for providing nursing orientation, working as the Epic support & Analyst, aiding in onboarding & credentialing in conjunction with the HR Department, functions as an agent of the CPI Program, teaching and training programs based on perceived needs, and continuing education for all nursing staff. Responsible for adhering to all LEC policies and procedures through maintaining employee Vaccines, Credentialing Status, CPR training, compliance training, and all other duties assigned.
Family Nurse Practitioner or Physician Assistant
The Family Nurse Practitioner or Physician Assistant (PA) is responsible for delivery of quality medical care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice.
Staff Physician
The Staff Physician is responsible for delivery of quality medical care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice.
Staff Physician (Pediatrics)
The Staff Physician is responsible for the delivery of quality medical care to patients of the Center and functions as a member of the multi-disciplinary healthcare team within the scope of his or her medical practice.

Since 1994, La Esperanza Clinic has been dedicated to providing high quality health and dental care to approximately 10,000 residents in San Angelo and the Concho Valley. With three brick-and-mortar clinics located on Buick Street, South Chadbourne, and 31st Street, as well as a Mobile Clinic that travels to rural areas, we strive to make healthcare accessible for everyone. Our mission is to deliver exceptional primary, preventative, and dental care, particularly to the medically underserved.

Student Preceptorship Program

La Esperanza Clinic offers hands-on preceptorships for healthcare students, providing real-world experience in a supportive, community-focused setting. Students learn alongside experienced providers while gaining valuable skills in patient care and teamwork.
