Duties and Responsibilities
Maintain and update patient records in the electronic health record (EHR) system.
Scan and electronically file incoming documents (labs, hospital reports, referrals, etc.).
Process requests for medical records releases in compliance with federal and state laws (HIPAA).
Ensure accurate documentation of information for audits, quality assurance, and legal purposes.
Track, log, and respond to external records requests (insurance, law firms, third parties).
Retrieve patient files for providers and staff as needed.
Monitor chart deficiencies and notify providers of missing or incomplete documentation.
Respond to phone calls and inquiries related to patient records in a timely manner.
Perform routine quality checks on scanned and uploaded documentation.
Assist with EHR data entry and record migration as needed.
Purge inactive or duplicate records according to retention policies.
Assist with front desk check-ins or clerical tasks during high-volume periods, if needed.
Knowledge, Skills and Abilities
Required
Knowledge of medical terminology and clinical documentation.
Proficiency with electronic health record (EHR) systems (e.g., eClinicalWorks, Epic, Athena).
Understanding of HIPAA and patient confidentiality guidelines.
Strong organizational skills with high attention to detail.
Ability to manage multiple tasks while meeting deadlines.
Excellent communication skills (verbal and written).
Basic computer literacy (Microsoft Word, Excel, Outlook).
Ability to work independently and as part of a team.
Preferred
Bilingual in English and Spanish is preferred but not required.
Credentials and Experience
Required
High School Diploma or equivalent.
Minimum of 1–2 years of experience in medical records or a healthcare setting.
Special Requirements
The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. This position does not require frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.
